The first two are joint first. In this list, there are a range of countries with equal number of access. So the USA is not in the top 30, let alone the top 10.

What do you mean?
If there is a race with 3 people and the first 2 come on at the same time, the last person is third, not second. The first two are joint first.
In this list, there are a range of countries with equal number of access. So the USA is not in the top 30, let alone the top 10.
